Insurance Policy

The patient is responsible for providing Breath of Life Mental Health and Wellness with the most recent, valid information regarding your insurance. At your initial visit, we will request your insurance card and make a copy for our records. Please bring your insurance card to every visit as we may require a copy to update your records. As a courtesy, we will submit your mental health claims to your insurance payers.

Insurance payouts could be decreased if all required information is not provided. Additionally, not all insurance plans offer coverage for all services. Some health plans choose which services they will reimburse. Your insurance provider will ultimately decide your eligibility and benefits. You will be responsible for the full cost of a service even if your health plan deems it to be “not covered.” Copays may be required by your insurance provider and are due at the time services are rendered. Please be aware that you will be invoiced for any outstanding balance if we are considered out-of-network for your coverage. Invoices are due upon receipt. If you are unable to pay the balance in full or have questions regarding your invoice, we encourage you to contact our office to speak with a representative.

No Show Policy

Patients who do not show up for an appointment or cancel their appointment with less than 48-hour notification will be subject to a cancellation fee of up to $50 depending on appointment type.

For no-shows and clients who arrive more than 10 minutes late, the appointment may need to be rescheduled, and a missed appointment fee may apply if we cannot accommodate you. In case of emergencies, we encourage you to contact our office as soon as possible, and we will make every effort to work with your situation. 

Patients who cancel repeatedly or are constantly late/ no-show may be subject to discharge.

Fees incurred for no-shows or late cancellations are the sole responsibility of the patients.
